WELFARE INCIDENT REPORT FORM

Please let management know if there has been an incident regarding your welfare, whilst working in the Club. This can either be anonymous or not, and will be followed up as soon as possible. If confidentiality is requested, then The Basement will ensure that this is the case. The incident will be handled fairly, sensitively and will be followed up with care. All feedback will be kept in confidence by the management team, unless it is necessary to conduct a wider team consultation, with your consent (in which case, anonymity will be maintained at all times).
